The problem statement is a clear and concise statement that describes the problem addressed in the self-change project. The following four guidelines are effective in creating a problem statement that is clear and concise:
I. Define the problem – define the problem in specific terms. Including elements from the evidenced based practice model such as client characteristics, circumstance, context, etc. (25pts)
II. Identify where the problem is appearing – Identifying where the problem is appearing, or manifesting, as specifically as possible. (25pts)
III, Describe the purpose or goal of the study. (20 pts)
IV. Give an evidenced based research question.
