Health disparities continue to create challenges in population health. Certain factors contribute to the likelihood of health disparities. It is important that as leaders within population health, you understand these factors and what can be done to help reverse health disparity trends. Eliminating disparities could reduce medical care cost by $230 billion and indirect cost of excess mortality and morbidity by $1 trillion over 4 years (Wilkinson, et.al., 2017).
You are the director of social services, and will be presenting to social services students at a local university. You are asked to explain key factors that contribute to health disparities. You should highlight how data driven approach impacts patients in the scope of population health messaging. With the move to a value-based care model from fee-for-service, there is a stronger connection between quality of care and payment. General Instructions: Students will determine whether or not their selected health program was effective or ineffective. What would you change or do differently next time? What advice would you give to the next person planning their health program?
The paper should be no less than one page and no more than two pages (not including the title page and references). Paper should be completed in 12-point font, double spaced with 1-inch margins in black ink with references if applicable.
In addition to including the above, make sure that you also mention
 Paragraph 1: Determine whether or not your program was effective or ineffective from papers 1-4 so far. “In order to ensure that ______ program was effective, an evaluation process will occur”… out of the two types in CH. 10, which program evaluation type will you use? Systemic or summative… formative or informal. State which type you will use and why you chose to use that type over the other.
 Paragraph 2: Determine what you would change about the planning process of your program- “If the program was granted additional funding, the program planners could have incorporated…” or “Next time, the team of program planners will…”
 Paragraph 3: What advice would you give to the next person planning their specific health program- What did you struggle most with? What was easier for you to complete within the planning process?
 Paragraph 4: How will your data be collected in order to successfully evaluate your program? – Quantitative or qualitative? Which method will you use to collect feedback? Don’t forget to mention the reason why you chose to evaluate your program and technique used for evaluating
Be innovative, think outside of the box and find something that you are passionate about to develop a program on- remember ethics when you consider your program requirements.
Grades will be determined based on all of the above details in your paper WITH explanations, APA formatting, grammar and overall organization. When citing information, in-text APA style citations MUST be included to avoid plagiarism. Things to avoid: Beginning sentences with “And” or “But”, run on sentences without a comma, incomplete sentences, spelling errors, using conjunctions like “we’re” or “I’m” and abbreviations, (be sure to spell out each word), including periods at the end of each sentence. Check and proofread for grammar sentences or wording that may be confusing.

The 4 Annotation questions I have provided here are the standard set that could apply to just about any source you would ever encounter. However, the responses to those questions largely depend on the category in which the source is located in your project. For instance, a Support source is backing up your basic position, so it should be as persuasive as possible and answer a research question as efficiently as possible. By contrast, a Counterargument source should not be persuasive when it comes to your position, and it should not be the ultimate answer to any research question. Your Annotations of these sources may change where they fit in these categories, so have an open mind as you weigh the arguments in all these sources. One of the biggest acts of revision for an Annotated Bibliography is a reassignment of the use and categories of these sources in this evolving project.

1) Be no less than five, but not more than 7 pages.
2) Have at least five (5) citations using APA style. All five reference should be from a refereed
journal. You are free to use additional references to support your paper, such as references
from websites and textbooks. Remember to include your citations in your reference section.
3) Cover the concepts stated in the outline given above.
4) It is recommended to finalize the analysis of the first facility before moving on to the second
facility.
